Signify Health, part of CVS Health®, seeks a Part-Time Physician to conduct In-Home Health Evaluations, engaging individuals in their homes to bridge gaps in care and build trust to improve health outcomes.
Responsibilities
- Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes
- Be part of efforts to visit millions of people nationwide with the goal of connecting them to the right care for their unique needs
- Conduct In-Home Health Evaluations lasting approximately 45 minutes, including a medication and medical history review, physical evaluation, and potentially additional diagnostic tests (if ordered)
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening, etc.)
Skills & Qualifications
Experience
- Active, unrestricted license(s) in coverage area(s)
- Multi-state licenses (preferred)
